The WPBA forms are designed for formative/summative assessment of specialty trainees’ competency but should be used as summative evidence for the Portfolio Pathway. The forms are traditionally used by CSRH trainees but can be adapted for this purpose; although you will not be able to complete educational supervisor details or add information relating to a training programme level, you should ensure that the complexity and level of competence shown reflects independent practice at specialist consultant level.
